Dear CHMA community,

We have two asks of you: one that will help our community continue to safely deliver to neighbors, and another that will allow us to self-reflect. Member Survey (estimated time: ~15-20 minutes)

After reaching our 6-month anniversary in September, we want to evaluate who we are, where we’re at, and where we might go in the future.

Here is the survey: nearly all questions are optional (but highly encouraged), and responses are anonymous. This information will be used for internal evaluation.

Please complete this as soon as possible; we would like to use preliminary information for our 10/24 Community Call. CHMA Waiver (estimated time: < 5 minutes)

In order to make sure that CHMA as a community is protected, we are asking all members to sign a waiver that establishes that delivery volunteers are liable for any harm that happens before, during or after a grocery delivery. Even though this is an organization run entirely by volunteers, we take this work seriously because we take our community’s wellbeing seriously. We are not a nonprofit or a formal organization. Because of that, we’re asking you to read and sign this agreement to protect us and the work we’re doing.
